Guardian Healthcare and Rehabilitation Center provides the opportunity to use the transition room in planning to return home. The transition room eases a rehabilitation resident’s release, while they remain supervised in our skilled nursing care center.

Physical, occupational, and speech therapies provide an interdisciplinary treatment plan targeting maximum independence through utilization of the transition room. During a resident stay in the transition room, therapy focuses on completion of activities of daily living including cooking, cleaning, laundry, and self-care, as well as independent ambulation and cognition to improve safety. Our transition room provides a home-like, comfortable setting. Our rehabilitation focuses on an interdisciplinary team approach with the goal of a successful return home.
